2022 Alumni Association Awards Program Nomination Form

We invite you to nominate alumni who, through their service and achievements, personify Empire State College’s tradition of excellence and have lived up to the college’s mission of transforming adult learners and their communities with diverse educational goals.

The nomination period is April 11, 2022 - August 15, 2022.
If multiple nominations are received for an individual, the most complete and representative one will be sent to the awards committee.
Nominees must be Empire State College graduates.

Nominating statements must be limited to two typewritten 8.5” x 11” pages.
Up to two professional letters of support must be submitted.
A curriculum vitae, resume or biographical sketch has to be included in the application.
